Pack up and return the equipment (US)

When your study ends, return the equipment as requested. Your Take-home kit and the electrode array that was on your head are both disposable – throw them away at the end of your study. 

Note:

Pack up and return the equipment (with shipping labels)

Once your study has ended,

1. Unplug Sight from power and place the charger and power cable into the Sense case.

2. Hold the base of the camera, open the bottom latch and lower the camera pole. Close the latch. Repeat for each latch until the camera pole is fully collapsed.

3. Remove the shipping labels from the Sense case. Apply the shipping label that says "12 lbs” to the handle on the Sense case. Apply the shipping label that says “33 lbs” to the Sight, on the side handle.

4. Pack the remaining pieces of equipment into the Sense case to protect them during transport:

  • iPad®, face down in the case
  • iPad charger
  • 2 Sight chargers
  • 2 Senses
  • Any patient diary or relevant notes if recorded on paper
